Bereits 1999 schrieb Mark Weiser in einem wegweisenden Artikel “The Computer of the 21st Century” über Tablets, das Internet der Dinge und Wearables.
Er prägte den Begriff Ubiquitous Computing – frei übersetzt Rechnerallgegenwart. Er schrieb:
“The most profound technologies are those that disappear.
“Technologien die den größten Einfluss auf uns haben, verschwinden vor unseren Augen.
Sie weben sich vollständig in unser alltägliches Leben ein, bis sie nicht mehr sichtbar sind. “
Nun berichtet Bloomberg, dass Apple arbeitet an einer erweiterten EKG Funktion für die Apple Watch arbeitet. Die Nutzer berühren mit 2 Fingern den Metallrahmen der Uhr und diese zeichnet ein Elektrokardiogramm auf. EKG Funktionen könnten in Zukunft zum Standard bei Wearables gehören. Damit rückt die Vision von Mark Weiser aus dem Jahr 1999 über Ubiqitous Computing ein Stück näher.
Das aktuelle Beispiel der Alternseinstufung von Apps im Google Play zeigt, dass es möglich ist, die Transparenz für Verbraucher zu verbessern und dabei in einem internationalen Markt die national unterschiedlichen Standards zu berücksichtigen. Das gibt Hoffnung, dass die App-Stores mittelfristig auch dem besonderen Informations- und Sicherheitsbedürfnis von Verbrauchern und Patienten im Markt der Gesundheits- und Medizin-Apps besser gerecht werden könnten. Ähnlich wie am Beispiel der aktuellen Altersklassifizierung von Google könnten die Entwickler von Medizin- und Gesundheits-Apps zukünftig ebenfalls dazu verpflichtet werden, anhand eines Standardfragebogens eine Einstufung der Vertrauenswürdigkeit der gesundheitsbezogenen Informationen und Services in Health-Apps vorzunehmen. Dieser könnte sich an den nationalen Anforderungen und den unterschiedlichen rechtlichen Rahmenbedingungen der jeweiligen Länder orientieren.
Abhängig davon, wie umfassend der Hersteller z. B. über die Ziele einer App, den Anbieter (Impressum, Kontakt), die Finanzierung, den Schutz der Nutzerdaten, die Werbepolitik, die Quellen und Autoren der gesundheitsbezogenen Informationen informiert, könnte automatisiert eine Einstufung auf einer Vertrauensskala erfolgen, die im besten Falle international abgestimmt und breit akzeptiert wird.
Wichtig wäre dabei, dass die Verbraucher – anders als dies derzeit bei der Alterseinstufung der Fall ist – auch die Angaben des Herstellers einsehen können, die zur Einstufung auf der Vertrauensskala geführt haben. Das wäre ein probater Weg, um mit überschaubarem Prüfaufwand ein großes Plus an Transparenz im Markt der Gesundheits- und Medizin-Apps herzustellen und Verbraucher bzw. Patienten in ihrer Medien- und Gesundheitskompetenz zu stärken.
Kriterien eines solchen Kataloges könnten z. B. die sieben Basiskriterien des Healthon-App-Ehrenkodex sein, die die Initiative Präventionspartner bei der Überprüfung von bisher über 350 Gesundheits-Apps bereits angewendet hat. Diese Kriterien orientieren sich an internationalen Standards (DISCERN, HON-Code). Perspektivisch könnte der Katalog um optionale Angaben erweitert werden, z. B. Informationen zu Wirksamkeitsstudien oder zu Usability-Tests bei Nutzerzielgruppen, die belegen, ob und in welchem Maße sich mit der Gesundheits- bzw. Medizin-App die ausgelobten Ziele erreichen lassen.
Fazit:
Auf Gesundheits- und Medizin-Apps liegen weltweit große Hoffnungen, die Qualität und Nutzerorientierung von Präventions- und Gesundheitsversorgungskonzepten zu verbessern. Vertrauen in diese neuen digitalen Helfer und die selbstbestimmte und risikoadaptierte Nutzung durch Verbraucher und Patienten, sind wichtige Voraussetzungen, um die Zukunftspotentiale von Digital Health gesamtgesellschaftlich verantwortlich zu nutzen. „Die Etablierung einer für Patienten und Verbraucher sichtbaren Vertrauensskala für Health-Apps, mit der jede Gesundheits-App vor ihrer Veröffentlichung in App-Stores anhand der Selbstangaben der Hersteller eingestuft wird, wäre ein pragmatischer Weg, der in einem internationalen Markt auch die Unterschiede in den nationalen Gesetzgebungen berücksichtigen könnte“, so Dr. Ursula Kramer, Initiative Präventionspartner.
