MS-Apps: Digitale Helfer für eine Krankheit mit 1000 Gesichtern
Anders als bei chronischen Erkrankungen wie Diabetes, Bluthochdruck oder Rückenschmerzen spielen Lebensstil und Ernährung in der Entstehung der Multiplen Sklerose keine Schlüsselrolle (1). Warum die Erkrankung häufig schon im jungen Erwachsenenalter auftritt und welche Einflussfaktoren die körperlichen und kognitiven Fähigkeiten in so unterschiedlicher Ausprägung einschränken, ist weitgehend unklar.
Wie sinnvoll kann da eine MS-App sein? Auf welche Weise soll sie den Betroffenen Hilfe bieten können?
Im Novemer 2015 hat die Initiative Präventionspartner das derzeitige Angebot für betroffene MS Patienten in Deutschland analysiert und untersucht, was diese Apps können und wie sie in Punkto Transparenz, Qualität und Vertrauenswürdigkeit abschneiden.
Hier die Ergebnisse:
- den rund 120.000 Betroffenen MS-Patienten in Deutschland (1) steht ein Angebot vonderzeit 8 MS-Apps zur Verfügung, die deutschsprachig und kostenlos über Google Play installiert werden können
- die Nutzerbasis der MS-Apps kann anhand der erreichten Downloads abgeschätzt werden. Nur eine App findet sich in der Kategorie bis zu 50.000 Downloads, die meisten Apps (n=6) erreichen max. bis zu 5.000 Downloads, eine App lediglich bis 500 Downloads
- Anbieter der Apps sind Selbsthilfeorganisationen bzw. medizinische Fachgesellschaften (n=3) sowie Anbieter von Arzneimittel zur Behandlung von MS (n=3).
Welche Unterstützungsfunktionen bieten MS-Apps?
- Informieren und aufklären über das Krankheitsbild (n=5; 63%)
- Krankheitssymptome der MS in einem Tagebuch aufzeichnen (n=5; 63%) und die Einträge graphisch auswerten (50%)
- Erinnern an die Einnahme bzw. Verabreichung von Medikamenten (n=4; 50%)
- Motivationsanreize setzen, um Tagebuchaufzeichungen regelmäßig einzutragen (n=4)
- Training der kognitiven Funktion (n=1)
Wie transparent informieren MS-Apps über den Schutz der Nutzerdaten bzw. die Quellen und Autoren der gesundheitsbezogenen Informationen?
- Die Mehrheit der MS-Apps informiert mit einer Datenschutzerklärung über den Schutz der personenbezogen Daten (n=5; 63%). Zum Vergleich: Betrachtet man alle bisher untersuchten Health-Apps unserer Datenbank so verfügt nur ca. jede vierte App (23%) über eine Datenschutzerklärung (2).
- Alle untersuchten Apps sind offensichtlich frei von Werbeeinblendungen.
- Angaben zu Autoren machen nur zwei der acht untersuchten Apps, Quellen nennt lediglich eine MS-App
Fazit: Die Multiple Sklerose ist eine Krankheit, die sich mit vielen verschiedenen Gesichtern zeigt. Eine App zu konzipieren, die für alle Betroffenen eine hilfreiche Unterstützung bietet, ist daher eine unlösbare Herausforderung. Deshalb bietet die DMSG (Deutsche Multiple Sklerose Gesellschaft) zusammen mit der AMSEL (Aktion Multiple Sklerose Erkrankter) drei verschiedene MS-Apps an: Eine App zur Information über das Krankheitsbild, zur Dokumentation der Krankheitssymptome sowie eine App mit Übungen speziell zur Stärkung der Kognition.
Ob die dauerhafte Aufzeichnung der Symptome und Defizite Betroffenen bei der Alltagsbewältigung helfen kann, und ob sich auf diese Weise Faktoren identifizieren lassen, die den Krankeheitsverlauf verschlechtern oder im anderen Fall eine Linderung herbeiführen können, bleibt fraglich. Das Führenn eines MS-Tagebuchs kann ein Versuch sein, durch eine bessere Informationsbasis eine solidere Grundlage zu bekommen, um z. B. in Abstimmung mit dem Neurologen Therapieanpassungen vorzunehmen oder bestimmte Stör- und Stressfaktoren zu erkennen und meiden zu können. Anleitung zu Kognitionsübungen lassen sich gerade per App videogestüzt und spielerisch sehr gut umsetzen. Die grundsätzliche Frage bleibt, ob man neurodegenerative Prozesse durch Übung kompensieren oder aufhalten kann oder ob die Defizit-Erfahrungen Betroffene nicht eher frustrieren und sich daher negativ auf die Gemütslage auswirken.

Vorsicht Pollenalarm! 21 Pollen-Apps im Test
In Deutschland leidet fast ein Drittel aller Erwachsenen an Allergien, die Hälfte davon klagt über Heuschnupfen (1). Eine wirksame Strategie für Allergiker ist es, den schädlichen Pollen aus dem Wege zu gehen. Und hier können Gesundheits-Apps wertvolle Dienste leisten. Welche das im Einzelnen sind, hat die Initiative Präventionspartner in ihrem aktuellen Screening untersucht und dabei insgesamt 21 deutschsprachige, kostenlose Apps für Pollenallergiker getestet. Was können diese Pollen-Apps?
- Aufklären: Zwei Drittel (62 %) der Apps klären auf mit gesundheitsbezogenen Informationen und Tipps, z. B. über verschiedene Pollenarten und wie man im Alltag mit Allergie besser umgehen kann.
- Erinnern bzw. warnen: Über die Hälfte der Apps (52 %) erinnern an die Einnahme von Medikamenten oder warnen mit einer Push-Nachricht, wenn die aktuelle Pollenbelastung einen vorher definierten Wert überschritten hat. Viele Apps nutzen dazu die GPS-gestützte Standortermittlung.
- Tagebuch führen: Fast jede zweite Pollen-App (48 %) hilft dem Nutzer dabei, ein Tagebuch zu führen um z. B. die aktuellen Beschwerden zu dokumentieren.
- Überblick geben: Wochen- oder Monatsdiagramme (52 %) helfen dabei, Trends zu erkennen.
- Allergierisiko einschätzen: Selbsttests (z. B. Allergie- bzw. Asthmatest) visualisieren das individuelle Risiko.
- Daten teilen: Jede dritte App (29 %) bietet dem Nutzer die Möglichkeit, seine Tagebucheinträge oder die Ergebnisse seiner Risikotests zu versenden, z. B. an den behandelnden Arzt.
Zwei Drittel aller Pollen-Apps sind sog. Multifunktions-Apps (66,6 %), d. h. sie bieten im Durchschnitt drei der oben genannten Unterstützungsfunktionen an.
Die 21 untersuchten Apps wurden in Google Play insgesamt max. 1,7 Millionen mal heruntergeladen, knapp 90 Prozent dieser Downloads gehen auf lediglich zwei Pollen-Apps zurück.
Fazit: Health-Apps mit einer Pollenvorhersage bzw. mit Informationen zu Pollen sind eine ideale Möglichkeit sich auch unterwegs über die aktuelle Pollenbelastung zu informieren. Das Problem: In Deutschland gibt es nur 14 Messstellen (2). Ein neuer Dienst des DAAB (Deutsche Allergie- und Asthmabund e. V.) setzt auf betroffene Allergiker, die die Belastung und die Schwere ihrer Symptome zusammen mit ihrem Standort melden können. Durch ein dichteres Datennetz soll die Güte der Pollenvorhersage verbessert werden. Leider ist dieser Dienst noch nicht als App verfügbar.

Ergänzende Hinweise:
- Drei der 21 getesteten Pollen-Apps bieten die Pollenvorhersage nur für die Schweiz, sie informieren jedoch auch über Allergien, Heuschnupfen oder Asthma, und können so auch für Allergiker in Deutschland nützlich sein, weshalb sie in diesem Screening berücksichtigt wurden.
- Fünf Apps sind Asthma-Apps bzw. Apps zur allgemeinen Gesundheitsvorsorge: Die Informationen zu Pollen bzw. die Pollenvorhersage ist lediglich ein Zusatzservice und nicht der eigentliche Schwerpunkt dieser Apps.
