In dieser Woche 141 Patientenkontakte und 14 Terminausfälle. Bis zum 10.9.2018 sind leider keine Termine mehr frei.

Sie kommen dann aber nicht in eine der nächsten offenen Sprechstunden, sondern tauchen ohne Anmeldung während meiner vollen Terminsprechstunde auf? Dann muss ich Sie leider wieder wegschicken, auch, wenn Sie sich vorwurfsvoll und abfällig äußern.

Diese Woche war ziemlicher Rummel um die mit Bagatellfällen überlasteten Notaufnahmen. Der eine fordert deswegen eine Notfallgebühr (oder auch nicht), der andere eine Ausweitung der Sprechstunden, aber bitteschön ohne finanziellen Ausgleich.
Manche halten eine Steuerung durch Zuzahlungen nicht für sinnvoll. Außer diesem und jenem (pdf; Vorsicht: führt zu einer berüchtigten Stiftung) habe ich auf die Schnelle nichts gefunden. Überlegungen zur Preissensitivität von Patienten gibt’s aber bei Niederegger.
Die seinerzeitige versteckte Beitragserhöhung der Krankenkassen, fälschlicherweise auch als “Praxisgebühr” bezeichnet, hatte zugegebenermaßen kaum einen Steuerungseffekt in den Praxen, wohl aber beim 14%-igen Anstieg der Notfälle nach Abschaffung der Kassengebühr. Vielleicht war das Ganze auch nur ein vordergründiges Ziel, um die versteckte Beitragserhöhung zu kaschieren.
Übrigens sollen die Krankenkassen ihren Pseudowettbewerb mittels Zusatzbeiträgen über die Preissensitivität ihrer BeitragszahlerInnen austragen. Die dürfen das also?
Ausgangsfrage ist doch, ob sich die Notfallversorgung über eine Notfallgebühr reformieren lässt.
Da ich mich in meiner Freizeit seit 15 Jahren immer noch gern mit Sozial- und Gesundheitspolitik befasse, möchte ich mein seit ebenso langer Zeit gebetsmühlenartig vorgetragenes Argument wiederholen: wir haben es (nicht nur in der Notfallversorgung) mit den Folgen einer impliziten Rationierung zu tun. Zu diesem Zweck wurden gesetzliche Vorgaben zur Mengenbegrenzung (mit dem ausdrücklichen Ziel der Beitragssatzstabilität) gemacht, die unmittelbar zur Folge haben, dass Termine beim Vertragsarzt eben ein begrenztes Gut sind. Und wenn dieses Fass überläuft, sucht sich der Patient den Weg des geringsten Widerstandes und läuft in die Notaufnahme des nächsten Krankenhauses, so wie Wasser eben bergab fließt.
In Psychiatrie und Psychotherapie besteht übrigens seit Jahren ein schier unglaublicher Nachfrageüberhang. Selbst wenn ich 80 oder 100 Stunden statt der üblichen 50-60 Wochenstunden arbeiten würde, wäre es nicht genug (und vor allem: ich müsste es unbezahlt tun). Ironischerweise werden die psychiatrischen Notaufnahmen nicht von so genannten Bagatellfällen überrannt – im Gegenteil. Teilweise müssen wir unsere Patienten sogar zum Jagen tragen…
Wenn ich aber höre und lese, dass Leute tatsächlich mit Bagatellen die Notaufnahmen fluten, frage ich mich schon, ob eine kleine Kostenbeteiligung die erwähnte Preissensitivität nicht doch ein wenig verbessern könnte. Gleichzeitig müsste man mal die bestehende Budgetierung bei den Vertragsärzten überdenken und damit einen Anreiz für zusätzliche Notfallbehandlungskapazitäten setzen.

It’s been some weeks since we’ve returned from one of Europe’s leading e-health events, namely the Doctors 2.0 & You conference in Paris, where we’ve met some of the brightest and smartest minds in e-health in Europe.
