Selbstversuch
Ich bin ja der Meinung, dass es gut für die ÄrztInnen/PatientInnen-Kommunikation ist, wenn man empathisch ist. Sich hineinversetzen kann. Vielleicht selbst mal krank im Spital gelegen ist. Um den Budenkoller nachvollziehen zu können.
Vor einem Jahr bin ich selbst operiert worden und klar, im Grunde wünsche ich niemandem eine OP, aber als Chirurgin selbst mal auf dem Tisch liegen zu müssen war eine aufregende und interessante, wenn auch hoffentlich einmalige Erfahrung. Man darf also ab Mitternacht nichts essen und trinken, muss nüchtern im Spital frühmorgens erscheinen. Aufgestanden bin ich natürlich trotzdem direkt zum Kühlschrank gegangen um ein Joghurt rauszuholen – STOPP! Gewohnheit. Nüchtern bleiben. So kommt man also nüchtern im Krankenhaus an, zieht sich dort komplett aus und schlüpft ein Baumwollnachthemd, welches hinten offen ist. Man kann es zwar mit einer einzigen Masche verschliessen, auf die Toilette schleiche ich trotzdem wie ein Krebs seitwärts mit dem Po Richtung Wand, damit nicht jedeR einen Blick auf meinen Allerwertesten werfen kann. Da es sich um eine Bauchoperation handelt, muss auch die Intimregion rasiert werden. Grund: die zu operierende “Fläche” wird im OP grossflächig desinfiziert, und dazu gehört auch dass man die Haare (potentielle Keimfänger) entfernt. Gut, Frau Menschenhandwerkerin, also ich, sitze rasiert, hungrig und etwas aufgeregt im Oma-Nachthemd im Bett und warte und warte und warte… Bis ich nach 2-3 Stunden in den Vorbereitungsraum der Anästhesie geschoben werde. Fühlt sich alles so normal an, in diesen Räumlichkeiten halte ich mich ja täglich auf. Bis mir der Anästhesist in die Vene sticht und mir einfällt – Hallo, ICH bin da heute ja die Patientin! Uarrrrgh! Der Anästhesist scherzt und labert rum, um mich abzulenken, dass ich an etwas Schönes denken soll wovon ich träumen möchte… Und weg war ich.
Heute geht es allerdings nicht um einen operativen Eingriff, sondern um ein in der Bauchchirurgie täglich vorkommendes Thema: das Abführen. Entweder im Rahmen der Vorbereitung für eine Darmspiegelung, oder weil man immens verstopft ist (z. B. aufgrund von Opiateinnahme). Manche PatientInnen haben leider die Arschkarte gezogen und müssen aufgrund von bestimmten Umständen öfter als normal zur Darmspiegelung bei uns antanzen und da wird, je nach Charakter, mal mehr oder weniger über das eklige Abführmittel geklagt. Davon müssen nämlich 24 Stunden vor der Untersuchung mindestens zwei Liter getrunken werden.
Die heutige Challenge lautet also…. MOVIPREP! (Nein, mir steht keine Kolonoskopie bevor, sondern diese Aktion dient ausschließlich der Empathie. Damit ich in Zukunft sagen kann: “Ich weiß, es ist echt eklig, ich habe das auch schon mal getrunken.” – oder: “Es schmeckt zwar nicht so gut wie Limonade, ist aber halb so wild.”.
Wie schreibt man so schön am Ende von Arztbriefen?

Glück im Unglück
Alternativtitel: Horrorvorstellung.
Frau A., jung und ansonsten gesund, kommt mit rechtsseitigen Unterbauchschmerzen auf den Notfall. Seit zwei Tagen habe sie die Beschwerden, ein bisschen Fieber, ansonsten bestünden keine Begleitsymptome. Keine Grunderkrankungen. Keine Voroperationen. Das CRP und die Leukozyten hoch. Im Ultraschall ein verdickter Blinddarm. Ich kläre die Patientin über das Krankheitsbild der akuten Appendizitis (Blinddarmentzündung) auf, sowie über die Therapiewahl Nummer 1 (Operation) und die damit verbundenen Risiken. Blutung, Wundinfekt, Verletzung angrenzender Strukturen, Laparotomie, Drainageeinlage. Eine Stunde später landet Frau A. auf dem Tisch. Ich führe die diagnostische Laparoskopie durch, die Appendix ist verdickt und klebt retrocoecal und ist auch noch perforiert. So weit so schlecht so gut. Eine Stunde später ist der Wurm draußen, die Patientin im Aufwachraum. Zwei Tage später geht Frau A. geheilt und ohne Schmerzen nachhause. Alles wie immer. Schema F. Ich frage mich, warum es genau Schema F heißt. Aso, preußische Frontrapporte also.
Drei Tage nach dem Eingriff trudelt der Pathohistologiebefund ein. Akute Appendizitis. Plus Tumor. T3. Die Frau ist wenige Jahre älter als ich. Scheiße.
Ich stelle den Fall beim Tumorboard vor, eine Hemikolektomie rechts, also die Entfernung des rechtsseitigen Dickdarms wird vorgeschlagen. Anschließend informiere ich den Hausarzt der Patientin. Ich drücke mich vor dem Telefonat mit der Patientin und führe das Gespräch zuvor mehrmals im Kopf durch, um mir zu überlegen, wie ich ihr diese Diagnose schonend beibringe. Schonend, funktioniert das überhaupt in diesem Zusammenhang? Was, wenn sie alleine zuhause ist und sich dann verzweifelt vom Balkon wirft? Soll ich es ihr lieber persönlich mitteilen? Wie soll ich sie vorzeitig in die Sprechstunde einbestellen, ohne dass sie Verdacht schöpft?
Letztendlich wähle ich ihre Nummer und erzähle ihr ruhig und sachlich, weswegen ich mich melde. Dass es mir leid tue, ihr so eine Nachricht überbringen zu müssen – und frage mich, ob so eine Aussage passend ist in so einer Situation? Sie erkundigt sich, wie es jetzt weiter geht und ich erkläre ihr, dass sie in die Sprechstunde kommen soll, damit wir den zweiten Eingriff planen können.
Es handelt sich um ein T3 Stadium, jedoch lagen noch keine Metastasen vor und ich freue mich für die Patientin, dass es immerhin durch die Blinddarmentzündung zur “Nebendiagnose” der Krebserkrankung gekommen ist, bevor Filiae entstanden.
